Winlevi is unique because it is the first topical treatment for acne that contains clascoterone, which is a novel anti-androgen agent. Sulfur and sulfacetamide are two commonly used topical medications for the treatment of acne and rosacea. Sulfur helps to unclog pores and reduce the formation of comedones (blackheads and whiteheads) by promoting the shedding of dead skin cells. It also has some anti-inflammatory effects, which can be beneficial for rosacea. Sulfacetamide is an antibiotic that belongs to the class of sulfonamides. It is effective against a wide range of bacteria, including Propionibacterium acnes, which is associated with acne breakouts. How to use sulfacetamide sodium-sulfur topical
This medication is for use on the skin only. Clean off make-up, oil and excess dirt from your face with a gentle cleanser before using this product. Wet the affected areas of skin before washing with this medication.
If your liquid form is a suspension, shake the bottle well before each use.
If you are using the liquid cleanser product, apply a generous layer of medication as directed by your doctor, usually once or twice daily. Gently massage it into the affected skin for 10 to 20 seconds until a full lather forms; rinse thoroughly and pat dry.
If you are using the cleansing cloth product, wash the affected areas with a cleansing cloth as directed by your doctor, usually once or twice daily. First wet your face with water. Then wet the cloth with a little water and work into a full lather. Cleanse your face with the cloth for 10-20 seconds. Do not get the lather into your eyes. Rinse thoroughly and pat dry.
If excessive drying occurs, you may need to rinse the cleanser off sooner or use it less often. Avoid getting any of this medication in the eyes, on the lips, or inside the nose or mouth. Consult your doctor or pharmacist for further details.
Side effects: sulfur odor, Redness, irritation or scaling of the skin may occur.

This combination medication is used to treat a certain type of acne (inflammatory acne vulgaris). It helps to decrease the number of acne pimples. Clindamycin is an antibiotic that stops the growth of acne-causing bacteria. Benzoyl peroxide can also stop the growth of bacteria as well as reduce oil production in the skin, helping keep your pores open.
During the first 3 weeks you are using benzoyl peroxide, your skin may become irritated. Also, your acne may seem to get worse before it gets better. If your skin problem has not improved within 4 to 6 weeks, check with your doctor.
WARNING: Benzoyl peroxide bleaches fabrics. Don’t get it on towels,
clothing, upholstery, pillowcases, linens, or carpeting.

How do topical retinoids work?
Topical retinoids work by regulating the turnover of skin cells. They help to unclog pores, reduce inflammation, promote collagen production, and encourage the development of new, healthy skin cells.
How should I apply topical retinoids? USE AT NIGHT
1. Cleanse: Begin by washing your face with a gentle cleanser and patting it dry.
2. Wait for dry skin: Wait for the skin to be completely dry before applying the topical retinoid. This helps to minimize irritation.
3. Apply a pea-sized amount to entire face using the 5 dot method: “5-dot” method. Start by placing a small pea-sized amount of the medication on your finger. Then, place “dots” in each of five locations of your face: Mid-forehead, each cheek, nose, and chin. Next, rub the medication into the entire area of skin - not just on individual pimples! Try to avoid the delicate skin around your eyes and corners of your mouth.

HOW TO PAIR WITH SPOT TREATMENTS FOR HYPERPIGMENTATION: After the retinoid, apply moisturizer to your entire face, followed by the dark spot treatment directly to the dark spot. Allow the dark spot treatment to dry.

What is Topical Metronidazole?
Topical metronidazole is a prescription medication used to treat the inflammation and redness associated with rosacea and periorificial/perioral dermatitis. It belongs to a class of drugs with anti-inflammatory and antimicrobial properties.How to Use Topical Metronidazole

Isotretinoin is a medication primarily used for the treatment of severe acne. It is a synthetic form of vitamin A and is sold under various brand names, including Accutane, Roaccutane, and others. Isotretinoin works by reducing the production of sebum, which is the oily substance that can contribute to acne formation. It is typically prescribed when other acne treatments have not been effective.

Prescribing and taking isotretinoin requires consent, registration and compliance with ipledge. Maintaining adherence requires monthly office visits to start and maintain therapy while on the medication. Failure to adhere may result in gaps of therapy and/or ineligibility of staying on the medication.
Additionally, while taking isotretinoin, it is crucial to:
Take the medication exactly as prescribed and at the recommended dosage.
Follow any additional instructions regarding food intake, as isotretinoin absorption can be affected by food.
Use effective contraception methods if you are sexually active, as isotretinoin can cause severe birth defects if taken during pregnancy.

Doxycycline, Minocycline, Erythromycin
Antibiotics decrease bacteria and inflammation, and are most effective for moderate-to-severe acne.
Minocycline, Doxycycline: Take one pill ONCE or TWICE per day, as instructed by your physician WITH FOOD.
Erythromycin, Azithromycin: Preferred in Pregnancy
NOTE: Always take these pills with lots of water. A pill stuck in the esophagus can cause significant burning and irritation. Avoid “popping” a pill right before bed and stay upright for at least one hour after taking a pill.
WARNING: Though less likely than doxycycline, minocycline may
increase your sensitivity to the sun, so practice excellent sun protection!
If you notice any of the following, stop using the medication and notify your health care provider: headaches; blurred vision; dizziness;sun sensitivity; heartburn-stomach pain; irritation of the esophagus; darkening of scars, gums, or teeth (more often with minocycline); nailchanges; yellowing of the eyes or skin (indicating possible liver disease);joint pains-and flu-like symptoms. Taking oral antibiotics with food may help with symptoms of upset stomach.

Opzelura (Ritlecitinib) is a Janus kinase (JAK) inhibitor medication used to treat moderate to severe atopic dermatitis in adults and adolescents. It helps with atopic dermatitis by targeting and inhibiting specific enzymes called JAK enzymes, which play a crucial role in the inflammatory process in the body.
Atopic dermatitis is a chronic inflammatory skin condition characterized by red, itchy, and inflamed skin. The condition is driven by an overactive immune response and an imbalance of certain cytokines (molecules involved in inflammation) in the skin.
JAK enzymes are intracellular signaling molecules that play a role in the transmission of signals from cytokines to the cell nucleus, leading to the production of pro-inflammatory substances. By inhibiting JAK enzymes, Opzelura reduces the signaling pathway that promotes inflammation in the skin, thereby helping to control the symptoms of atopic dermatitis.

Usage: Tacrolimus ointment helps reduce inflammatory skin reactions. It is prescribed for people with moderate-to-severe eczema, usually as an alternative to other treatments such as steroid creams or ointments. Can be used for additional inflammatory conditions: psoriasis, periorificial dermatitis and other rashes. It is applied topically (on to the skin) to reduce symptoms such as inflammation, redness and itching. Tacrolimus ointment is available in two strengths: 0.03% and 0.1%. Only the lower, 0.03% strength, is suitable for use by children. It will be prescribed initially by a skin specialist doctor.
Tacrolimus is also available as a medicine which is taken by mouth, but this is prescribed for a completely different medical condition.
Side effects: While tacrolimus ointment is generally safe and well-tolerated, some people may experience mild side effects such as skin irritation, burning, or itching at the application site. These effects are usually temporary and resolve on their own.
Recommend use for tacrolimus: twice daily as needed
Application: Wash your hands thoroughly before applying the ointment. Gently clean and dry the affected skin area before applying a thin layer of tacrolimus ointment. It is important to cover only the affected skin and not use it on unaffected areas.
On application, it may cause some sensitivity, therefore I recommend mixing the ointment with aquaphor.

Cysteamine is a molecule that has been studied for its potential use in treating hyperpigmentation disorders, including melasma. Sente is a skincare brand that has developed a product called "Sente Cysteamine Cream" specifically for the treatment of melasma.
Cysteamine works by inhibiting the production of melanin, the pigment responsible for skin color. In the context of melasma, excessive melanin production contributes to the development of dark patches on the skin. By reducing melanin production, cysteamine aims to lighten these patches and improve the appearance of melasma.
How to Use:
Step 1:
Apply to clean dry skin. If cleansing before use, wait at least 20 minutes until skin is completely dry.
Step 2:

Tranexamic acid is an antifibrinolytic agent that is commonly used in medicine to prevent or control excessive bleeding. In recent years, oral tranexamic acid has been shown to be effective in melasma by reducing the production and activity of melanocytes, the cells responsible for producing the pigment melanin. By inhibiting melanin production, tranexamic acid may help to lighten the dark patches associated with melasma.
Contraindications: hormonal therapy (birth control, hormonal replacement therapy), active cancer, history of clotting (blood clot DVT, stroke, heart attack) and/or genetic increased risk of clotting.
How to use: Take one-half of a 650 mg tablet by mouth twice daily

Oral Minoxidil was first approved by the FDA for the treatment of high blood pressure in 1979 under the trade name Loniten. Johnson & Johnson then developed topical minoxidil (Rogaine) in a 2% and 5% topical forms.
How does minoxidil work? Minoxidil dilates small blood vessels and helps stimulate hairs to return to the growth phase. It can be used for a variety of types of hair loss, including pattern baldness, telogen effluvium, CCCA and others.
Side effects: The most common side effect is itching and irritation of the scalp, at sites of application of the medication. Initial shedding in the first few weeks of using the medication. Do not be alarmed, this is actually a GOOD SIGN and hair growth will commence after the shedding phase.
Stopping minoxidil: If you discontinue the minoxidil, the results will not be maintained, but your hair will NOT fall out more or worse than without using the medication at all.
Oral minoxidil: Over the past several years, clinical trials have been published confirming the efficacy in low dose oral minixodil in treatment hair loss. The use of oral minoxidil for hair loss is off-label (has no FDA approval) but is widely used by board certified dermatologists with success.
How does oral minoxidil work? Oral minoxidil is a vasodilator, meaning that it helps relax the blood vessels and thereby increase the vascular supply to the hair follicles.
Who should NOT take oral minoxidil? Any patient with high blood pressure, heart disease, pheochromocytoma, renal failure, any history of any cardiac disease. Pregnant women.
Risks of oral minoxidil: facial or body hair or hypertrichosis (most common), water retention (lower leg swelling), light-headedness from low blood pressure, difficulty sleeping.

The U.S. Food and Drug Administration (FDA) approved this medication to treat male pattern hair loss however has also been discovered to be helpful for additional types of hair loss. It belongs to a group of medications called 5-alpha reductase inhibitors which converts part of your body’s testosterone into dihydrotestosterone, or DHT — the primary culprit behind male and female pattern hair loss. Decreasing DHT decreases miniaturization of the hair follicle.
Oral Finasteride (Propecia®) 1 mg tablets: Take 1 tab once a day. Taking it at the same time each day seems to produce the best results. You can take this medication with or without food. Do not take your medication more often than directed.
Risks of oral finasteride:
Erectile dysfunction (ED), decreased libido, ejaculation disorder, breast tendereness, decreased libido, rash and/or depression. (<4%)
Topical finasteride: has been proven to decrease DHT levels to improve hair loss and stimulate hair follicles. However, does not impact serum DHT levels in the body as much as oral finasteride and thus the risk of sexual side effects is very low.
Risk profile of topical finasteride: very mild, redness of the skin, local irritation, contact dermatitis
Special considerations:
Men + PSA levels: This medication can interfere with PSA laboratory tests for prostate cancer. If you are scheduled to have a lab test for prostate cancer, tell your care team that you are taking this medication.
Pregnant women: Women who are pregnant or must not handle broken or crushed finasteride tablets, or topical compounds with finasteride. The active ingredient could harm the unborn baby.

This medication is a diuretic, which has been prescribed for decades to treat hair loss.
Dosing: Higher doses (200 mg) are associated with a greater treatment response.
Monitoring: Electrolytes and blood pressure
Reported side effects: Irregular menstruation, Frequent urination, Dizziness, Headaches, Nausea, Vomiting, Breast tenderness, Breast enlargement
Combination therapy: Research has shown that the combination of minoxidil and spironolactone can be helpful in treating thinning hair. One study testing the usage of 5% minoxidil with 80 to 100 milligrams of spironolactone showed a significant increase in hair shaft diameter.
